Uninsured services Services not insured by OHIP
There are some services we provide which are not covered by OHIP, and for which you are therefore responsible for payment. Payment is due at the time the service is completed.
Such services include the completion of sick notes, insurance forms, travel medicine counseling, cosmetic procedures of the skin, and third party requests for physical examinations (i.e.: work, camp, driver’s license, etc.). In some cases both the forms and associated physical exam are uninsured. The fee will depend on what service is provided and the amount of time necessary to complete it.
Some of the more common services and fees can be found on this page. We follow the Ontario Medical Association Schedule of Fees in setting our uninsured rates. Please feel free to discuss these with doctor if you have any concerns about your ability to pay. Please note that fees for forms do not include the cost of the associated medical examination, if any is required.
Transfer of your Medical Record to another facility or physician:
- $40 per individual, $70 per family. A larger fee may need to be assessed if your physician must undertake a special review or extra paperwork.
Work and school related forms/notes:
- Back to work note, Sick note, Day care note (free of communicable disease): $15
- Federal Employee Absence Form (blue): $22.43
- Tuberculosis (Mantoux) testing: $44.48
Summary of a medical record (typed letter):
- billed hourly according to time taken by physician
Third Party Physicals for:
- Schools and camps: $22.43 (plus examination fee)
- Pre-employment certificates, Fitness Clubs, Nursing Home or Hospital employees: $29.93 (plus examination fee)
- Driver's Medical: $168.04 total (includes form and examination)

- Certificate of Health Practitioner (form OCF-8): $36.71
- Physician statement of Ongoing Disability: $40

- Certification of incompetence (financial): $234.09 including assessment to determine such
Health Advice / Immunization for purpose of travel:
- $40 plus cost of immunizations
Cosmetic Procedures of the skin (including treatment of common warts, excluding plantar and venereal warts):
- Liquid nitrogen $20
- Removal of other benign lumps and bumps $80 or hourly rate, as determined by physician
Botox injections for pain or cosmetics:
- $7/unit for pain management
- $10/unit for cosmetics
Urine pregnancy test - express diagnostics: $10
Allergy vaccines: flat fee: $160 per course
Photocopying (miscellaneous) - $0.5 per page
Uninsured Vaccinations
- Prevnar (infant meningitis, caused by Pneumococcus): $110/dose; one to four doses may be required, depending on age at first vaccination.
- Menjugate (meningitis, caused by Neisseria meningitides Type C): $70/dose; one to three doses may be required, depending on age at first vaccination.
- Varivax (chickenpox): $80/dose; one dose for child under 12 years old, two doses for adults.
- Vaqta (Hepatitis A): $70/dose; two doses required
- Recombivax (Hepatitis B): $40/dose; three doses required
- Twinrix (Hepatitis A and B): $75/dose; three doses required
